Baby constantly scratching his face?

My three month old is constantly scratching his face and making it bleed. I think he does it in his sleep. I trim his nails whenever they seem sharp and/or long. He hates to have mitts on his hands so they are not an option. I am so afraid he is going to have scars on his face from the scratches. Any suggestions? Can I file his nails?
